- 司马懿 review 驳回后评论 `@zhangfei-dev 请注意边界检查`——张飞虽然会收到 review_result Mail,但不知道 specifically @了他

§13.3.2 已有 `extract_mentions()` 初步设计,但只覆盖了 `issue_comment` 场景,且没有和工具链自动流转做去重/互补分析。本设计做完整端到端覆盖。
